Only Legal, Financial and Mental health professionals are eligible for membership in Collaborative Practice Professionals of Illinois. At this point please create a user name and password and provide your email address. Once you have completed your application you will be a member pending approval. Members who are approved and completed their public profile will be included in the public directory and searches on the site.
Spaces are allowed; punctuation is not allowed except for periods, hyphens, and underscores.
A valid e-mail address. All e-mails from the system will be sent to this address. The e-mail address is not made public and will only be used if you wish to receive a new password or wish to receive certain news or notifications by e-mail.
Provide a password for the new account in both fields.
Professional Licenses
You must have at least one of these licenses/designations. If you have more, by all means include each of them.

You must have taken at least a 2-day basic Training in Collaborative Practice. You will be able to add other trainings you've taken as part of your profile.

You must be or become a member of the International Academy of Collaborative Professionals. For more information about the IACP, visit http://collaborativepractice.com. By joining CCPIllinois you receive a discount on the IACP Membership. Upon completing the other approval guidelines you will need to provide your IACP member number on your Public Profile.
